ENROLLMENT INFORMATION 


New Families:  Please make an appointment with our District Registrar, Suzy Stone.  Call 816-690-5262 before coming in to set up your appointment.     

Please complete Online Registration in order to enroll students for the 2024-25 school year.  ALL parents will need to provide proof of residency each year.  You will able to upload your residency documents when you are completing the online registration process.  Please read the "Proof of Residency" link for more information.

All Students

       (Only need to bring in as a new student or if new immunizations have been given)

      Drug Testing Form - High School Only  This is also in your online registration.

Enrollment Documents Required for New Students

The primary parent/guardian who lives in the Oak Grove R-VI School District must be present when a student enrolls. New families to the district must make an appointment with our District Registrar, Suzy Stone at 816-690-5262.

Print and bring the following completed forms to enrollment:

Kindergarten and students transferring from other schools should bring the following documents to enrollment:

  • Certified Birth Certificate 
  • Immunization Record 
  • Proof of Residency 
  • Proof of Custody for minor child, if applicable (example: Divorce Decree)
Age/Birth Certificate: 

Age must be verified by a Certified Birth Certificate, not a hospital birth record. By law students may enroll for school if their birth dates occur in the following manner:

  • Kindergarten students must be five (5) years of age BEFORE August 1. 
  • First grade students must be six (6) years of age BEFORE August 1. 

Immunization: 
According to Missouri statute, it is unlawful for any student to attend school unless he/she has been immunized as required under the rules and regulations of the Division of Health of the Department of Public Health and Welfare, and can provide satisfactory evidence of such immunization. It is unlawful for any parent or guardian to refuse or neglect to have his/her child immunized, as required by this section, unless the child is properly exempted.
